
Bilingual, Spanish Advanced Practice Provider β Cardiology

This is a fully remote position, open to applicants in Alabama, +4 more states.
β’ Perform comprehensive assessments for patients with general cardiology and heart failure requirements through virtual consultations, which includes ordering necessary diagnostics, interpreting laboratory and imaging results, and collaborating with the specialty care clinic team to create a treatment plan.
β’ Develop precise diagnoses and personalized treatment strategies for individuals with heart failure and various cardiology conditions, incorporating medication management, volume monitoring, and lifestyle adjustments.
β’ Initiate, adjust, and oversee guideline-directed medical therapies (GDMT) for a diverse group of patients suffering from heart failure.
β’ Proactively manage heart failure and related conditions while addressing potential obstacles to optimal care to reduce the risks of heart failure exacerbation and hospital admissions.
β’ Evaluate and intervene in weight variations and acute volume overload management for patients enrolled in the clinic.
β’ Assess, diagnose, and manage various general cardiology conditions appropriate for virtual care, including stable coronary artery disease, hypertension, hyperlipidemia, stable atrial arrhythmias, peripheral arterial disease, and chronic venous disease.
β’ Order in-home diagnostics, such as echocardiograms, EKGs, and Holters, and collaborate with community cardiology practices for additional diagnostics, including stress testing and advanced cardiac imaging.
β’ Work closely with other care team members, including PCPs, cardiologists, and other Devoted team members like pharmacy, clinical nursing, and social work, as well as interacting with family members and caregivers to coordinate holistic care for the member, ensuring continuity and a collaborative care plan.
β’ Act as the clinical advisor and provide clinical escalation support to the specialty clinic staff and other teams during business hours.
β’ Utilize our proprietary electronic health information system for consultations while also providing feedback to enhance the interface.
β’ Ensure accurate and current patient medical records, complying with all relevant legal and ethical standards.
β’ Engage in quality improvement initiatives and continuous professional development to remain updated on best practices and advancements in cardiovascular care.
β’ Comply with all pertinent laws, regulations, and industry standards, including patient privacy and telehealth regulations.
β’ NP or PA with a minimum of 5 years of experience in cardiology clinical practice, ideally with 2 years in an outpatient cardiology environment.
β’ Bilingual in English and Spanish.
β’ Experience conducting visits via telehealth video platforms and a strong commitment to practicing virtually, supporting the mission of delivering care where the patient resides.
β’ A secure and private workspace equipped with a reliable internet connection.
β’ High proficiency with technology, including EMRs, messaging platforms, online portals, and AI chat.
β’ Availability to work from 8 AM to 5 PM in the Central Time zone or later time zones (MT, PCT).
β’ Willingness to obtain and maintain multistate licensure within the first 90 days (with support and reimbursement provided).
β’ Familiarity with managed care is advantageous, including the ability to assess STARS/HEDIS measures, code clinical comorbidities, and identify clinical care gaps.
β’ An active and clear RN and APRN or PA license in Texas, Ohio, Arizona, Illinois, Georgia, or Alabama, along with APRN certification, is required at the time of hire and must be maintained during employment at Devoted Medical.
β’ Cardiovascular Nurse Practitioner Certification is a plus.
