Bilingual Registered Nurse – Virtual Care, Care Management (Spanish/English)

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee the care of medically complex patients while creating and monitoring care plans across various programs.

• Prepare charts for Annual Wellness Visits (AWV).

• Perform thorough nursing assessments that address physical, functional, psychological, social, environmental, and educational needs.

• Update primary care providers and interdisciplinary care teams on patient status and care plan modifications.

• Ensure patient records are maintained accurately, promptly, and confidentially.

• Bilingual in English and Spanish is a must.

• Must hold an active and unrestricted Registered Nurse (RN) license in a compact state.

• At least 2–3 years of clinical nursing experience is required.

• Experience in providing care through telehealth or virtual platforms is essential.

• Experience in remote work settings is preferred.

• Familiarity with EHR systems and telehealth technology is necessary.

• Strong skills in clinical assessment and care coordination are required.

• Excellent verbal and written communication abilities are essential.
