Bilingual Nurse Case Manager – Mandarin

This is a fully remote position, open to applicants in Canada.

📋 Description

• Oversee and implement services for Sentrex Health Solutions patient support initiatives.

• Manage all elements of the designated program support and serve as the primary liaison for assigned healthcare providers.

• Ensure that patients have access to support programs and medication therapies.

• Engage with patients, insurance providers, pharmacies, physicians, and other healthcare professionals to secure medication coverage and manage patient cases.

• Execute healthcare provider and patient registration and enrollment processes.

• Process and verify program data entries for accuracy and integrity.

• Respond to telephone and web-based inquiries regarding program services, disease states, and products.

• Offer medical assistance, disease and therapy education, and disease management recommendations.

• Attend specialized training sessions conducted by pharmaceutical teams.

• Provide inbound and outbound call support, including maintenance and adherence follow-ups.

• Administer case-management protocols, including clinical monitoring, data collection, follow-ups, compliance checks, therapy disruption/discontinuation monitoring, and adverse event reporting.

• Collaborate with reimbursement specialists and physicians to secure therapy funding and explore alternative funding sources.

• Participate in patient meetings and conferences as necessary.

• Report adverse events to manufacturers and medical information teams, entering them into required systems within designated timelines.

• Uphold confidentiality regarding patient, client, and corporate information.

• Coordinate with physicians’ offices, pharmacies, laboratories, infusion/injection service providers, and other stakeholders to manage patient services and resolve issues.

• Engage in continuing education activities.

• Create training materials and programs for new members of the patient support program team.

• Schedule laboratory monitoring, facilitate coverage renewals and modifications, and provide reimbursement assistance.

• Review, analyze, and confirm patient information for completeness and accuracy.

• Schedule appropriate next steps to ensure seamless service delivery.

• Follow up and advocate until coverage decisions are finalized.

• Support drug shipment requests.

• Carry out additional tasks as assigned by the manager.

⛳️ Requirements

• Must hold an RN or RPN designation in good standing with the College of Nurses of the respective province.

• Prior experience in Patient Support Programs is essential.

• Knowledge of reimbursement processes for obtaining patient coverage is an advantage.

• Experience in Hepatitis is considered a strong asset.

• A minimum of 2 years of clinical experience is preferred.

• Bilingual proficiency in English and Mandarin is required.
