Bilingual, Haitian Creole/English Transitions of Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• The Care Coordinator plays a crucial role in supporting TOC Clinical Operations to ensure that our patients have suitable post-discharge planning and follow-up established.

• This position involves assisting the team with patient outreach and coordination during the patient's inpatient stay and after discharge.

• Engaging with patients while they are hospitalized to introduce the TOC program and communicate the follow-up care from TOC clinicians.

• Organizing post-discharge follow-up appointments (PCP/Specialty).

• Contacting hospitals and provider offices to collect clinical documentation.

• Processing Home Care and DME orders along with vendor follow-up.

• Overseeing the return fax queue and verifying provider profiles for updated contact and fax information.

• Offering additional operational support to the assigned team and manager as needed.

• Ensuring patients are ready for telehealth visits by assisting with technology preparations.

• Collaborating internally during clinical rounds to discuss patients at risk for readmission.

• Coordinating lab tests and imaging, and tracking results to guarantee that they are reviewed by the ordering clinician.

⛳️ Requirements

• A minimum of 2 years of experience in patient-facing roles and care coordination within healthcare, managed care, home health, and/or community health settings.

• Experience in a telemedicine environment.

• Fully bilingual in English and Haitian Creole.

• Experience in Managed Care / Health Plans is preferred; MA Certification is desired; expired MA certifications are acceptable! [Verification: A copy of your active or expired certification will be required. If you do not have a certification but possess significant MA experience/background, we may ask for copies of your MA school diploma and transcripts.]
