Bilingual Diabetes Educator – Certified Diabetes Care and Education Specialist

This is a fully remote position, open to applicants in United States.

📋 Description

• Evaluate members for eligibility and suitability for the Diabetes Clinic.

• Conduct thorough assessments and follow up with members to provide ongoing education regarding the disease process, medications, glucose monitoring, lifestyle counseling, and contingency planning.

• Arrange follow-up appointments for both yourself and providers.

• Connect members and providers with appropriate resources within Devoted to address clinical issues.

• Establish and work towards collaborative SMART goals with members.

• Utilize motivational interviewing techniques to promote positive and lasting behavioral changes.

• Recognize gaps in care and offer relevant resources.

• Work in conjunction with nurse practitioners, social workers, and pharmacists to create care plans and optimize medication regimens.

• Keep accurate records related to interactions with members and providers.

• Continue to enhance your skills through educational series and continuing education opportunities.

⛳️ Requirements

• Active unrestricted Compact Registered Nurse license or unrestricted Registered Dietitian license.

• Ability to obtain additional licenses in states where Devoted operates, as required.

• Current Certified Diabetes Care and Education Specialist (CDCES) certification.

• Minimum of 2 years of experience in the clinical management of diabetes.

• Remote experience in diabetes management is highly preferred.

• Familiarity with diabetes technology, including continuous glucose monitors (CGMs) and insulin pumps.

• Bilingual capability in Spanish and English.
