This is a fully remote position, open to applicants in Illinois.
• Establish consistent communication with assigned oncology members through phone, text, email, and video calls to provide ongoing support and follow-up.
• Assist and collaborate with the RN in delivering member care by liaising with providers and pharmacies, as well as facilitating referrals to third-party infusion services for cost containment.
• Identify, coordinate, and facilitate referrals to infusion services, clinical resources, or supportive care programs.
• Address social determinants of health (SDOH) that impact members' access to care, medication adherence, and overall well-being.
• Offer education and guidance regarding treatment logistics, medication access, and community or program resources, reinforcing physician-directed care.
• Provide emotional support and connect members to counseling, support groups, survivorship resources, and other supportive services.
• Collect, document, request, and transmit member information in the EHR in compliance with HIPAA; escalate clinical concerns to the RN when necessary.
• Monitor member interactions and assist the RN in achieving enrollment, engagement, and quality metrics.
• Work collaboratively with the RN and interdisciplinary team to ensure timely and coordinated oncology care for members.
• Active LPN/LVN license in good standing within a compact state, with a willingness to obtain licensure in additional states as needed.
• A minimum of 4 years of experience as an LPN/LVN, along with clinical experience in oncology and infusions, is required.
• Strongly preferred experience in care navigation, treatment support, and/or complex case management.
• Must be flexible with work hours to support members across all US time zones, including some evening and weekend availability.
• Fluency in both spoken and written Spanish is required.
• Preferred experience in Utilization Review and/or Care Management.
• Highly preferred experience working 100% remotely as a nurse.
• Willingness to travel occasionally, which may involve overnight stays for corporate events, conferences, and health fairs.
• Ability to attend and engage professionally in video meetings.
• Strong technological skills, with the capability to effectively use computers, peripheral equipment, and applications/systems, including Microsoft products.
• An autonomous self-starter who is comfortable navigating ambiguity.
• A creative mindset with the ability to appropriately challenge existing norms.
• Excellent written and verbal communication skills.
• A proactive approach to overcoming obstacles with a ‘yes if…’ mentality.
• Ability to effectively manage competing deadlines while maintaining focus on the larger goals.
