This is a fully remote position, open to applicants in California.

📋 Description

• Develop cases, tasks, and ensure thorough documentation in the Case Management module for all Hospital and SNF discharges.

• Contact members via phone to provide assistance with referrals, authorizations, home health care (HHC), durable medical equipment (DME), medication refills, and scheduling provider appointments and follow-ups.

• Request and upload medical records from primary care physicians (PCPs), specialists, hospitals, and other healthcare providers, including discharge summaries.

• Collaborate with the RN Case Manager to engage and manage a panel of members.

• Monitor new alerts and inform the Case Manager of any changes in condition, admissions, discharges, or new diagnoses.

• Complete and document tasks assigned by the nurse.

• Build relationships with members, gain their trust, and serve as a patient advocate.

• Raise concerns with the nurse if members show signs of non-compliance or experience changes in their condition.

• Contribute to outreach activities for members across all levels of case management programs.

• Help maintain and update member records.

• Assist in mailing or faxing communications to members, primary care physicians (PCPs), and/or specialists.

• Meet established deadlines by prioritizing tasks in accordance with department policies, standards, and business requirements.


⛳️ Requirements

• At least 1 year of experience in care coordination, case management, or transitions of care within a health plan, IPA, MSO, or medical office environment.

• Experience in supporting members/patients with scheduling, authorizations, referrals, and coordinating services (e.g., home health, DME, follow-up care).

• Background in directly interacting with patients/members in a telephonic or care coordination setting.

• Required: High School Diploma or GED and/or four (4) years of relevant experience in lieu of education.

• Preferred: Bachelor's degree.

• Required: Bilingual in English and Spanish.

• Proficient computer skills, with the ability to type 35 WPM using 10-key touch (Microsoft Outlook, Excel, Word).
