Bilingual Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• The Evergreen Chronic Care Management (CCM) Care Coordinator facilitates proactive care coordination services for patients dealing with complex chronic conditions.

• Engage in patient outreach, develop, sustain, and reinforce established care plans.

• Ensure compliance with documentation and coordinate communication among patients, providers, and healthcare partners.

• This position is primarily outreach-oriented and focused on operational tasks.

• Experience in supporting nephrology or chronic kidney disease (CKD) patient populations is highly preferred.

• The program prioritizes SMS as the primary channel for patient engagement, with phone outreach utilized as necessary.

⛳️ Requirements

• Certified Medical Assistant

• Bilingual (Spanish/English)

• 1–3 years of experience in healthcare

• Experience in chronic disease or nephrology is preferred.

• Strong skills in clinical assessment, communication, and documentation.

• Comfortable working across two systems (EHR + CCM platform) simultaneously.

• Capable of managing a structured, metrics-driven workflow with consistency.

• Intermediate proficiency with the MS Office Suite, including Outlook and Teams.

• Ability to work effectively in a predominantly remote setting:

• Home internet must meet a minimum download speed of 25 Mbps and upload speed of 10 Mbps.
