Bilingual Advanced Practice Provider – Complex Geriatric Care

This is a fully remote position, open to applicants in Arizona, +3 more states.

📋 Description

• Provide outstanding care to a diverse group of complex patients across various locations, utilizing video telehealth consultations and a collaborative interdisciplinary team.

• Direct and manage the medical treatment of patients (employing the 5M framework) throughout their journey with Gold Care.

• Collaborate closely with other members of the patient’s healthcare team, including their primary care physician, specialists, and other Devoted teams such as pharmacy or specialty care.

• Engage with the patients’ support networks, including family members and caregivers, to coordinate care effectively and implement a unified care plan.

• Ensure the maintenance of accurate and current patient medical records, adhering to applicable legal and ethical standards.

• Comply with all relevant laws, regulations, and industry standards, particularly those concerning patient privacy and telehealth practices.

⛳️ Requirements

• A valid and clear RN and APRN or Physician Assistant license in AZ, CO, HI, and/or OR is essential at the time of hire and must be sustained throughout employment at Devoted Medical, with a commitment to obtain and maintain multiple state licenses.

• 3-5+ years of experience as an APP in primary care, house calls, internal medicine, family medicine, geriatrics, and/or palliative care.

• Familiarity with telehealth technology and electronic health records (EHR) is required.

• Active BLS certification at the time of hire, which must be upheld during employment at Devoted Medical.

• Proficiency in both English and Spanish is preferred for this position, though not mandatory.
