Behavioral Health Social Work Case Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Assist in coordinating and supporting the mental and physical health care needs of patients within an assigned caseload.

• Conduct screenings and assessments for prevalent mental health and substance abuse disorders while promoting patient engagement and follow-up care.

• Deliver brief behavioral interventions utilizing evidence-based techniques.

• Monitor patient follow-up and clinical outcomes by maintaining a registry.

• Engage in regularly scheduled consultations regarding the caseload with the psychiatric consultant.

• Aid in the management of psychotropic medications as prescribed by medical providers.

• Facilitate the coordination of care with available resources as necessary and offer social work case management.

⛳️ Requirements

• Education: Must be a Licensed Clinical Social Worker, Licensed Professional Counselor, or licensed in a related field of study.

• Completion of a degree along with all post-degree supervision required for licensure, and a minimum of 2 years of post-degree experience.

• Proven experience in effectively providing evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders.

• Capability to engage patients effectively in a relatively short-term therapeutic relationship through video and/or audio communication.

• Demonstrated high level of organizational, collaborative, and communication skills.

• Licenses: Must be licensed in one or more states, with the willingness, education, and experience necessary to obtain multiple state licenses.
