This is a fully remote position, open to applicants in Pennsylvania.
• Serves as an integral member of the core care team to create tailored clinical pathways or best practice initiatives for specific patient populations.
• Engages in practice staff meetings and training sessions.
• Offers education and support on a range of subjects, leveraging expertise in behavioral health and substance abuse to assist multidisciplinary team members.
• Takes part in daily huddles, focusing on identifying behavioral health, substance abuse, and psychosocial requirements.
• Delivers population-based care utilizing a registry for high-risk/high-need patients.
• Provides services to all enrolled individuals, not limited to those with identifiable mental health disorders.
• At least a Master’s degree in a Mental Health-related field.
• A minimum of 2 years of experience applying evidence-based counseling methods, including screening, diagnosing, and treating prevalent mental health and substance use disorders.
• Must hold a valid Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT) credential.
• CPR certification from the American Heart Association is required.
• Act 34 Criminal Background Clearance Certificate is necessary.
• Act 33 Child Abuse Clearance Certificate is essential.
• Act 73 FBI Fingerprinting Criminal Background Clearance Certificate must be obtained.
