Behavioral Health Case Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Utilizes relevant screening criteria knowledge and clinical expertise to evaluate member needs.

• Performs assessments to determine individual requirements and formulates a care plan to achieve the objectives and goals identified during the assessment.

• Oversees and assesses the effectiveness of the care plan, making adjustments as necessary.

• Facilitates member access to suitable, high-quality, and cost-effective care.

• Collaborates with internal and external resources to address the identified needs of members, working closely with providers.

⛳️ Requirements

• Holds a MA/MS in social work, counseling, or a related behavioral health discipline, along with a minimum of 3 years of clinical experience in social work counseling, particularly with a diverse range of complex psychiatric and substance abuse disorder treatments.

• Possesses a current, active, unrestricted license such as RN, LCSW (as applicable by state law and scope of practice), LMHC, LICSW, LPC (as permitted by relevant state laws), or LMFT.

• Alternatively, a Clinical Psychologist is required to practice as a health professional within the boundaries of licensure in the relevant states or territories of the United States.

• Preferred experience includes case management and coaching (telephonic and/or in-person) with members facing a wide array of complex psychiatric and/or substance abuse and medical disorders.
