This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Assist in the General Case Management Process within the Case Management Department.

• Initiate the outreach cycle and document in electronic records and/or track member referral status.

• Monitor, manage, and prioritize member needs within the established intervention period.

• Provide education and coaching to members, families, and/or caregivers regarding follow-up care, health status, transition care goals, and health insurance benefits.

• Ensure timely coordination of healthcare services.

• Help members with scheduling appointments, tests, screenings, and transportation services.

• Communicate effectively with external stakeholders and vendors to coordinate member care as necessary.

• Accurately input data into MedHok, Casetrakker, Jiva, and other relevant databases.

• Support the design and execution of programs pertaining to quality measures.


⛳️ Requirements

• High School diploma is mandatory.

• An Associate’s degree in business or a related administrative or clinical field is preferred.

• A minimum of 3-5 years of experience in a professional setting is required.

• Experience within a healthcare environment, particularly in managed care, is preferred.

• Strong understanding of Clinical Services departments and Point32Health organizations.

• Proficient computer skills, particularly in Microsoft Word, Excel, email programs, and the Intranet.

• Familiarity with PowerPoint, Access, Visio, and additional software applications is desirable.

• Excellent written and verbal communication skills are essential.

• Ability to recognize emergency situations and adhere to established protocols.

• Strong organizational skills with capacity to manage multiple demands.
