This is a fully remote position, open to applicants in Texas.
• Manage cases for families of children with complex medical needs who are part of Imagine Pediatrics’ behavioral health program.
• Oversee high-risk pediatric patients up to 21 years old, including those recently discharged, ensuring proper follow-up care.
• Conduct comprehensive biopsychosocial assessments that address behavioral, social, emotional, and systemic needs.
• Develop, assess, and adjust care plans for patients and their families.
• Provide interventions during mental health crises, behavioral challenges, and family conflicts.
• Coordinate case management and support across various systems for mental health diagnoses and concurrent medical conditions.
• Connect families with validated external resources such as social service agencies and behavioral specialists.
• De-escalate situations and stabilize patients and family members during crisis events.
• Offer psychoeducation regarding mental health diagnoses, treatment adherence, and related topics.
• Collaborate with schools, specialists, child welfare agencies, and other members of the external care team.
• Engage in interdisciplinary consultations to track patient progress.
• Act as a representative of Imagine Pediatrics to patients, families, providers, and the community.
• Carry out additional responsibilities as assigned by the manager.
• A Master’s degree with major coursework in social work or a related field is required.
• An LMSW certification is mandatory.
• A minimum of 3–5 years of postgraduate experience in healthcare social work or case management within behavioral health is required.
• Experience with pediatric populations and family systems is essential.
• Strong skills in motivational interviewing, goal setting, intervention, and needs assessment.
• Knowledge of social work practices, including crisis prevention and intervention.
• Experience delivering telehealth services.
• Proficiency in MS Office Suite and ability to navigate online platforms.
• In-depth understanding of behavioral health principles and practices.
• Expertise in trauma-informed care practices.
• Strong understanding of common signs and symptoms of pediatric/adolescent mental health conditions and the ability to recognize coping difficulties.
• This is a remote position with 10% travel required for training and educational purposes.
• Flexibility to work afternoons and evenings is necessary.
• Bilingual Spanish is strongly preferred.
