Behavioral Health Care Coach

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Evaluate and assess the needs of members to create individualized care plans.

• Coordinate services and oversee the implementation of case management plans in alignment with members’ personal objectives.

• Identify and address obstacles to effective care.

• Ensure that members receive services in the most appropriate and least restrictive environment for their needs.

• Design and execute interventions that address care requirements.

• Assist members and their families in navigating suitable care and community resources.

• Facilitate communication with payer sources, providers, interdisciplinary teams, and other stakeholders involved in members’ care.

• Travel 75–90% of the time to visit Medicaid members in homes, assisted living facilities, long-term care facilities, and various other care environments.

• Work Monday through Friday, from 8:00 AM to 5:00 PM, totaling a 40-hour work week.

• Participate in mandatory TB screening and occasional travel to Humana offices for training sessions or meetings.

⛳️ Requirements

• A bachelor's degree in a health and human services discipline.

• Must be a resident of South Carolina.

• Minimum of 2 years of relevant experience.

• Outstanding verbal and written communication skills, along with strong interpersonal capabilities.

• Proficient in various electronic information systems and software applications.

• Must have a separate room with a locking door that can serve as a home office to ensure continuous privacy.

• Preferred experience with Medicare and Medicaid recipients.

• Familiarity with electronic case note documentation and multiple computer systems/applications is preferred.

• Experience in health promotion, coaching, and wellness is preferred.

• Knowledge of community health and social service agencies, along with additional community resources, is preferred.

• Ability to maintain a 40-hour work week, Monday through Friday, from 8:00 AM to 5:00 PM.

• Required to undergo TB screening if selected for the position.

• A valid state driver's license is necessary.

• Must maintain personal vehicle liability insurance that meets the state's minimum requirements or Humana's specified limits, whichever is greater.

• Must have home internet service that meets minimum download speeds of 25 Mbps and upload speeds of 10 Mbps; satellite, cellular, or microwave connections require leadership approval.

• Must work from a dedicated area free from interruptions to safeguard member PHI/HIPAA information.
