This is a fully remote position, open to applicants in Illinois, +1 more state.
• Lead teams comprising nurses and behavioral health professionals tasked with care management.
• Supervise the assessment and evaluation of members’ needs to attain or sustain optimal wellness.
• Assist members and families in accessing suitable care and wellness resources.
• Direct the development, implementation, and evaluation of care management programs that align with organizational goals and regulatory standards.
• Manage multiple managers or highly specialized professional associates.
• Oversee data sharing among internal and external resources to guarantee service integration and continuity of care.
• Foster communication and collaboration with providers, payers, regulatory agencies, and other stakeholders.
• Spearhead the preparation and presentation of program reports.
• Utilize data analytics to track performance, identify trends, and propose enhancements.
• Advocate for process improvement and innovative initiatives in care management.
• Travel to provider offices, Illinois HFS and CMS, Humana Illinois locations, and occasionally to other Humana offices for training or meetings.
• Must reside in Illinois, Indiana, or Wisconsin.
• Active, unrestricted RN, LCSW, or LCPC license in Illinois.
• A minimum of six years of professional experience in healthcare or managed care.
• At least three years of experience with a managed care health plan.
• Minimum of two years in a leadership role managing leaders or supervisors.
• Three or more years of experience leading care coordination teams and achieving measurable outcomes across diverse member populations.
• Expertise in analyzing and interpreting data trends.
• Progressive business consulting and/or operational leadership experience.
• Ability to travel within Illinois to provider offices, Illinois HFS, CMS, and Humana Illinois locations.
• Preferred: advanced degree in nursing, Social Work, or a business-related field.
• Preferred: experience with MLTSS and HealthChoice populations.
• Preferred: previous experience in managed care.
• Preferred: five or more years of management/supervisory experience.
• Capability to work from a dedicated space without ongoing interruptions to safeguard PHI/HIPAA information.
• Self-provided internet service with a minimum of 25 Mbps download and 10 Mbps upload speed.
