This is a fully remote position, open to applicants in New Jersey, +3 more states.
β’ Conduct Patient Screening and Manage Intake: You will reach out to patients and inquire about their health using a predetermined script. These interactions typically last between 15 to 20 minutes. Your objective is to create a welcoming environment where patients feel at ease sharing their truthful responses.
β’ Connect Patients with Resources: You will ask probing questions to ascertain each patient's needs. Subsequently, you will provide relevant resources and refer them to health and mental health services.
β’ Maintain Accurate Records: During each conversation, you will document patient responses in an Electronic Health Record (EHR) system. You will review and finalize your notes by the end of each workday, ensuring compliance with federal privacy regulations (HIPAA) and billing standards. You will also gather and record patient consent at the beginning of each call.
β’ Organize Your Schedule: You will manage your own daily calendar, make reminder calls to minimize missed appointments, and achieve weekly objectives. Additionally, you will schedule follow-up calls for patients and allocate time between appointments to complete your notes.
β’ Education: A high school diploma or GED is mandatory. A degree in Human Services, Public Health, or a related field is advantageous.
β’ Languages: Proficiency in both English and Spanish (or another language) is highly preferred.
β’ Certification: A Community Health Worker (CHW) certificate is preferred. If you do not possess one, you must be willing to obtain it through training offered by Wider Circle and/or by completing a state certification application.
β’ Experience: Experience in healthcare, social services, or community work is beneficial but not essential. You should be comfortable communicating with diverse individuals over the phone. Comprehensive training will be provided.
β’ Tech Skills: You should be proficient in using websites and applications for data entry and tracking. Adaptability in a fast-paced work environment is essential.
β’ Self-Directed: You should be capable of managing your own responsibilities, staying organized with notes, and achieving goals independently.
β’ Training: You must be willing to participate in our onboarding program, which includes call practice, documentation tasks, and role-playing. A final test must be passed with a score of 80% or higher.
β’ Background Check: You must be willing to undergo a drug test and background check prior to commencing work.
β’ You should be able to sit, stand, and walk as required. Adequate vision and hearing to communicate with patients both by phone and in person is necessary. You must also be capable of typing and utilizing multiple computer programs simultaneously while speaking.
β’ A reliable internet connection and a quiet, private environment for patient calls are required.
β’ Pay: $18β$20 per hour
