• Deliver integrated support to both patients and healthcare providers through a hands-on approach aimed at enhancing the quality of patient care.
• Promote high-quality pharmacotherapy by ensuring the delivery of safe, effective, and cost-efficient drug therapy.
• Work in close collaboration with the hybrid ACP team to fully leverage the comprehensive suite of therapy management (TM) clinical services available in supported clinics.
• Conduct in-person and/or virtual phone assessments regarding disease state management and drug therapy education, categorized according to medication risk levels and approved clinic protocols.
• Aim to identify, resolve, and prevent issues related to medication.
• Provide additional services to address barriers to medication adherence and any related challenges.
• Monitor and evaluate medication regimens to confirm that clinical criteria are satisfied, including therapeutic objectives, side effects, and drug interactions.
• Spot opportunities for optimizing drug therapy and recommend medication cost-saving strategies when appropriate.
• Engage in quality improvement initiatives by participating in clinical outcome studies, REMs compliance/reporting, and ACP-related process enhancement projects.
• Streamline resources and workflows by supporting roles that manage complex prior authorizations/appeals, providing clinical justification for medication approvals, including relevant literature and rationale.
• Collaborate with healthcare professionals, including assigned clinic physicians, specialists, and onsite clinic pharmacists, to ensure effective medication management and coordinated care.
• Candidates must possess a Doctor of Pharmacy degree (PharmD) from an accredited pharmacy school.
• Must hold a valid pharmacist license in CO.
• Additionally, candidates must meet at least one of the following criteria:
• Hold certification in a relevant practice area.
• Have completed a postgraduate residency at an accredited institution, with at least 50% of the experience involving direct patient care services alongside interdisciplinary teams.
• Have delivered clinical services to patients for a minimum of one year under a collaborative practice agreement or protocol with a physician, advanced practice pharmacist, or pharmacist engaged in collaborative drug therapy management, hospital pharmacy practice, or health systems.
• Candidates must fulfill the required continuing education obligations to maintain their CO state pharmacist license if selected for the position.
• Strong analytical skills, sound judgment, excellent verbal communication abilities, extensive knowledge of pharmacology advancements, and understanding of FDA health regulations are essential. Candidates should also demonstrate the ability to manage processes effectively, exhibit robust technical and clinical skills, maintain a focus on legal compliance, and be highly productive with a commitment to quality and detail. A solid clinical background and proven expertise in disease state management and treatment protocols are crucial.
