This is a fully remote position, open to applicants in California.

📋 Description

• The Office Coordinator for the Assisted Living Waiver Program Care Coordination Agency (ALWP CCA) is tasked with processing beneficiary applications in accordance with the Regulatory Agency and Libertana Home Health Policy.

• Aids Medi-Cal beneficiaries in gaining access to waiver services, state plan services, and various community resources by identifying, coordinating, and gathering necessary data for the ALWP application.

• Ensures the accurate completion of all documentation required for the State Waiver Program Application in line with ALWP and Agency policy.

• Reviews and assesses ALWP documents essential for the State Waiver Program Application, which includes Initial Assessments, Re-assessments, ISPs, Physicians Reports, and Medi-Cal eligibility.

• Engages in discussions regarding State inquiries or requests for documentation with the Lead ALF Coordinator, ALF Manager, ALF Staff, SNFs, and other referral sources.

• Responsible for compiling Patient Charts in accordance with Agency policy.

• Establishes and nurtures positive relationships with RCFE, SNFs, and other vendors.

• Aids the Agency in adhering to Federal, State, Local, HIPAA Regulations, and Joint Commission Standards.

• Supports facilities and ALF Care Coordinators in ensuring compliance with Habilitation Hours.

• Guarantees that assessments are conducted and all requisite documents are finalized.

• Addresses Client, RCFE, SNF, and vendor-related issues.

• Follows up with RCFE managers, discharge planners, and other relevant client contacts, fostering working relationships and educating them about the ALW Program and its available services.

• Demonstrates sound judgment and knowledge of contemporary ALF Care Coordination practices and procedures in alignment with Libertana Policy and DHCS ALWP Policy.

• Engages in relevant continuing education as required by your immediate supervisor.

• Ensures a pleasant work environment for all employees.

• Adheres to all agency policies and procedures.

• Respects the rights and property of patients and families as outlined by federal and state laws.

• Driving may be necessary to reach geographical areas serviced by the company.

• Possesses knowledge of Confidentiality, HIPAA, and applicable healthcare laws and regulations.

• Reports any incidents of Fraud and Abuse.

• Has knowledge of mandated reporting requirements.

• Performs additional duties as assigned.


⛳️ Requirements

• Familiarity with the State of California Assisted Living Waiver Program Care Coordination Agency Provider Handbook.

• Knowledge of the RCFE Provider Handbook.

• Understanding of Title 22 Division 6 for RCFE.

• Exhibits excellent judgment and possesses strong organizational skills.

• Fluent in English.

• Demonstrates effective written and verbal communication abilities.

• Proficient in using computers, electronic medical records, and Microsoft Office applications.

• Capable of establishing and maintaining effective communication and relationships with office, field, and administrative personnel.

• Self-motivated with the ability to work independently with minimal supervision.

• Must possess a valid driver's license and have access to an insured automobile that is in good working condition, following state and/or organizational standards.

• A Bachelor’s degree in healthcare, business administration, or a related field is preferred.

• Previous experience in healthcare is preferred.

• Background or knowledge in report development, newsletters, statistics, and information analysis is desired.

• Exhibits strong analytical skills.
