This is a fully remote position, open to applicants in Florida.
• Overseeing and managing complex chronic comorbidities.
• Designing and executing treatment plans tailored for patients and their family caregivers.
• Collaborating with the transdisciplinary team to create individualized care plans.
• Tracking patients’ progress and adjusting care plans as necessary.
• Working alongside other healthcare professionals to ensure comprehensive and coordinated care.
• Keeping abreast of the latest trends and advancements in geriatric care.
• Conducting screenings and assessments for social determinants of health (SDOH) and behavioral needs.
• Delivering exceptional customer service.
• Providing clinical and medical education to both clients and staff.
• Engaging in virtual care conferencing.
• Updating and maintaining medical records.
• Participating in quality improvement initiatives and contributing to the establishment of best practices.
• Aiding in the strategic direction of Nolia Care services, a leader in the industry.
• Employing strong interpersonal, leadership, and problem-solving skills, along with a dedicated commitment to providing excellent, team-oriented care.
• A minimum of two years of outpatient experience in an independent private practice or within a larger medical facility.
• Preferred experience in Family Nurse Practitioners (FNPs) and Adult Geriatric Primary Care Nurse Practitioners (AGPCNP).
• Required experience in utilizing chronic, longitudinal care models in digital health.
• Must possess Nurse Practitioner and Registered Nurse licenses in the state of Florida (additional licensure in CA, TX, ID, WA, MI, or multi-state is also required).
• Board certification as a Nurse Practitioner in geriatric care is preferred, and DEA certification is required.
• National board certification as a Nurse Practitioner from an accredited organization.
• A Master’s level ARNP is required, with added training or experience in Geriatric care, Dementia, and Palliative specialties.
• A Doctorate in Nursing is preferred.
• In-house billing support to assist with coordinating benefits, eligibility, and claims submissions.
• An in-house scheduler available to manage calls and arrange intake appointments.
• Consistent individual clinical supervision.
• Regular case consultations.
