This is a fully remote position, open to applicants in Pennsylvania.
• Deliver diagnostic patient care mainly through virtual consultations via video calls or phone.
• Act as the decision-making authority for clinical calls during after-hours and weekends.
• Evaluate, diagnose, treat, and document the physical and psychosocial health status of patients.
• Assess both acute and non-acute clinical issues through virtual video consultations or telephone.
• Conduct and record physical evaluations and patient histories.
• Analyze trends in patient conditions and create, document, and implement management plans.
• Formulate care plans that include health education, physician referrals, case management referrals, and counseling for patients and families.
• Organize patient care based on knowledge of the patient population and established protocols.
• Gather and assess comprehensive and focused health data for both patients and their families.
• Collaborate with primary care providers and other medical professionals to report health data and ensure compliance with guidelines.
• Guarantee optimal patient outcomes through telemedicine and work with on-call primary care providers as necessary.
• Monitor progress towards expected outcomes and apply appropriate protocols.
• Improve telehealth practices and systems by utilizing data that demonstrates program effectiveness.
• Communicate using diverse formats, tools, and technologies to provide care throughout the continuum.
• Offer safe, effective, financially responsible services along with exceptional customer service.
• Engage in clinical rounds, conferences, risk and quality management programs, and relevant meetings.
• Comply with departmental objectives, performance standards, regulatory mandates, quality-care regulations, policies, and procedures.
• Operate in accordance with a written or electronic practice agreement.
• Contribute to the development of telehealth/telemedicine policies, procedures, and protocols.
• Initiate or be a part of quality improvement initiatives.
• Participate in committees that support organizational growth.
• Provide constructive feedback on others’ practices to enhance patient care.
• Coordinate services with other programs.
• Carry out additional duties as assigned by management.
• Bachelor’s degree in Nursing (BSN) and graduation from an Advanced Practice Nursing program with certification in the specialized area is required.
• A Master’s degree in Nursing is mandatory.
• Board certification from AANP or ANCC is required.
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross must be obtained upon hire.
• A current DEA number for Schedule II-V controlled substances may be necessary based on the state of practice.
• Multi-state licensure is required, including Florida and at least three licenses from VA, GA, MI, MO, OH, PA, TN, TX, IL, KY, LA, and KS.
• A minimum of 3 years of clinical work experience in acute/primary care is required.
• Experience in telehealth is preferred.
• Proficiency in both spoken and written English is essential.
• The ability and willingness to achieve independent/autonomous practice as a Nurse Practitioner in applicable states is necessary.
• Proficiency in Microsoft Office Suite, including Excel, Word, PowerPoint, and Outlook is required.
• Willingness to travel up to 10% for meetings and training sessions is necessary.
• Willingness to travel to Georgia or other states for onsite observation and medical record review is required.
• Availability to work evenings, overnights, and/or weekends based on the assigned schedule is needed.
• A minimum commitment to work four holidays each calendar year is necessary.
• Capability to conduct telemedicine visits in accordance with state and federal regulations is essential.
• Expert knowledge of clinical functions, practices, processes, procedures, techniques, and methods is required.
• Knowledge of medical practices to operate independently as a certified practitioner and collaborate with other medical providers is important.
• Demonstrated success in achieving clinical performance metrics is necessary.
• Excellent clinical judgment and problem-solving skills are required.
• Ability to prioritize tasks and work effectively under pressure is essential.
• Strong communication and collaboration skills with physicians, patients, and team members are necessary.
