This is a fully remote position, open to applicants in Illinois.
• Fulfill credentialing and privileging requirements for inpatient core privileges, as suitable for the position.
• Participate in didactic lectures and educational opportunities.
• Deliver direct patient care under the guidance of a clinical preceptor.
• Collaborate and consult with physicians and other healthcare professionals.
• Manage a designated patient caseload.
• Enhance communication and presentation skills with the interdisciplinary team.
• Increase knowledge of common disease processes.
• Recognize professional development needs and create learning opportunities with preceptors and mentors.
• Carry out comprehensive patient health assessments, histories, and advanced physical examinations.
• Order and interpret laboratory and diagnostic tests.
• Order or prescribe medications, therapies, interventions, and procedures within clinical privileges and PA scope of practice.
• Document care and procedures in the medical record.
• Develop and direct personalized, comprehensive treatment plans.
• Coordinate discharge planning and after-visit care.
• Consult with specialties and disciplines to address patient healthcare needs.
• Engage in committees, task forces, and professional organizations.
• Advocate for patients, families, nursing staff, and members of the healthcare community.
• Assist in the development of clinical policies and procedures.
• Identify, assess, and resolve clinical system and process issues in collaboration with leadership.
• Formulate, implement, and evaluate quality improvement strategic plans using evidence-based guidelines.
• Work with physicians and nurses to spearhead changes in clinical practice.
• Educate and counsel patients and families on preventive care, medical conditions, care plans, outcomes, treatments, and medications.
• Provide both formal and informal education to internal and external groups.
• Review and apply evidence-based research and guidelines to clinical practice.
• Participate in service-line quality initiatives and the development of outcome metrics.
• Perform other Advanced Practice Provider duties as assigned.
• Complete a 12-month multidisciplinary Primary Care fellowship encompassing clinical rotations, didactic curriculum, and supervised hands-on care.
• APRN-CNP: Graduation from a recognized Nurse Practitioner program.
• APRN-CNP: Fulfillment of all required qualifications for the APRN-CNP job description.
• Master’s degree from an accredited school/college of nursing.
• Registered Professional Nurse (IL) license.
• Advanced Practice Registered Nurse (IL) license.
• Adult-Gerontology Nurse Practitioner certification.
• Basic Life Support for Healthcare Providers certification from the American Heart Association.
• PA: Graduation from an accredited PA program.
• PA: Fulfillment of all required qualifications for the PA job description.
• PA (IL) license.
• Preference for current or past NM employees.
• Interest in Primary Care, particularly in hypertension, weight management, and geriatrics is preferred.
• Completion of credentialing and privileging requirements for inpatient core privileges, as suitable for the role.
