Advanced Practice Provider – Complex Geriatric Care

This is a fully remote position, open to applicants in Ohio, +1 more state.

📋 Description

• Provide outstanding care to a diverse group of complex patients across various locations, utilizing video telehealth consultations and a multidisciplinary team approach.

• Direct and manage the medical care of patients (employing the 5M framework) throughout their time with Gold Care.

• Collaborate closely with other members of the patient's healthcare team, including their primary care physician, specialists, and additional Devoted teams such as pharmacy or specialty care.

• Work in partnership with the patients’ support networks, including family members and caregivers, to coordinate patient care and implement a collaborative care plan.

• Conduct thorough assessment visits, offer post-acute transition support, perform comprehensive diagnosis/disease evaluations, medication reviews, and assess quality of care (Stars/Hedis) interventions, along with social and home health/DME requirements.

• Keep precise and current medical records for patients, ensuring adherence to relevant legal and ethical standards.

• Comply with all applicable laws, regulations, and industry norms, including those concerning patient privacy and telehealth practices.

⛳️ Requirements

• A valid and clear RN and APRN or Physician Assistant license in GA, PA, and/or OH is required at the time of hiring and must be maintained throughout employment at Devoted Medical, along with a willingness to obtain and uphold licenses in multiple states.

• At least three years of experience as an APP in primary care, house calls, internal medicine, family medicine, geriatrics, and/or palliative care.

• Competence in utilizing telehealth technology and electronic health records (EHR).

• Current BLS certification required at the time of hire and must be maintained during employment at Devoted Medical.

• Preference for candidates with virtual care experience and a strong commitment to continuing clinical nursing practice and conducting virtual visits, believing in the mission of delivering care where patients reside.

• Experience in providing clinical care in collaboration with interdisciplinary teams is preferred.

• Familiarity with working with complex, frail, or vulnerable populations, especially within a Skilled Nursing Facility (SNF) environment.

• A team-oriented mindset, commitment to continuous improvement, and a positive attitude.

• Experience in a fast-paced, high-energy work environment.

• Ability to quickly adapt and remain flexible with new processes.

• Proficiency in both English and Spanish is preferred for this role, but not mandatory.
