Advanced Practice Provider, APP

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Deliver comprehensive clinical care through text and video formats, covering Acute Care, including pediatrics and mental health.

• Availability to work on Mondays and Fridays with flexible starting hours (e.g., 7 am-12:30 pm ET).

• Conduct video consultations for Acute Care, Primary Care, and Behavioral Health patients.

• Assist physicians with Epic inbox management, including labs, imaging, and patient inquiries.

• Provide education to patients regarding suitable treatments and care plans tailored to their health needs.

• Prescribe medications and refills as deemed appropriate.

• Focus on delivering high-quality care that prioritizes patient outcomes.

• Enhance remote care by offering compassionate and meaningful support.

• Utilize critical thinking to create innovative solutions to emerging problems or issues as necessary.

• Work collaboratively with Care Team Members.

• Strive to achieve challenging goals as part of a top-tier team.

⛳️ Requirements

• A minimum of 5 years of clinical practice experience as a Family Nurse Practitioner or Physician Assistant in a Primary Care environment.

• Must hold a valid license in New Jersey.

• At least 1 year of experience with Epic EMR is required.

• Currently enrolled or eligible for enrollment as a Medicare provider.

• Experience in primary care and behavioral health, with the ability to treat both acute and chronic conditions.

• Should possess technical proficiency, be proactive, organized, detail-oriented, and have experience with telemedicine.

• A clean background and medical malpractice history are essential.

• Must be willing to commit to 40 hours per week, with 36 of those being clinical hours.
