Active Care App Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Oversee clinical and operational aspects of a team of Advanced Care Providers (APP).

• Provide exceptional primary care services to members of the Devoted Health Plan.

• Collaborate closely with the primary care leadership team and your team of APPs.

• Establish a leading virtual primary care practice, enhanced by advanced AI-driven technology.

• Analyze performance trends and visit quality through dashboards, documentation reviews, and direct observation.

• Work in tandem with primary care leadership to outline the essential elements of the Active Care Service Vision.

• Oversee team capacity, manage schedule coverage, and track visit volume trends.

• Conduct a limited number of clinical visits weekly, adjusting as per staffing and program requirements.

⛳️ Requirements

• Previous experience in primary care with a strong background in management.

• A minimum of 3 years of NP clinical experience in relevant healthcare settings.

• Proven ability to lead change initiatives effectively.

• Track record of supporting, mentoring, or developing healthcare professionals.

• Excellent communication skills and the ability to navigate change with ease.

• Experience working collaboratively in interdisciplinary teams.

• Proficient use of the Google suite of applications.

• Valid, unrestricted RN and APRN licenses, along with national certification.

• Willingness to acquire additional state licensure with assistance.
