This is a fully remote position, open to applicants in New York.
β’ Lead a diverse team of approximately 9 professionals, including nursing, substance use, vocational, family, and peer specialists, along with a psychiatric provider, while adhering to the OMH ACT model and maintaining a 10:1 participant-to-staff ratio.
β’ Conduct the Daily Morning Meeting; provide individual and field-based supervision based on evidence-based practices.
β’ Establish referral connections with SPOA, hospitals, and community partners; manage a measured admission pace to reach full census.
β’ Oversee compliance for participants under court-mandated AOT and collaborate with the county AOT team.
β’ Supervise documentation quality, CAIRS reporting, chart audits, and the annual fidelity self-assessment; leverage PSYCKES and program data to enhance quality improvement.
β’ Ensure 24/7 clinical consultation availability and participate in the team's rotating on-call schedule.
β’ Master's degree with valid NYS licensure (LCSW, LMSW, LMHC, etc.); OR NYS-licensed RN; OR licensed physician.
β’ Over 5 years of experience working with adults facing serious mental illness, including at least 2 years in a supervisory role.
β’ Background in a multidisciplinary community-based team (e.g., ACT, AOT, mobile crisis, intensive care management).
β’ Proven expertise in evidence-based practices such as motivational interviewing, harm reduction, and trauma-informed care.
β’ A valid driver's license (agency vehicles are available); pre-employment clearances are required, including fingerprinting and a check against the NYS Justice Center Staff Exclusion List.
