
Weekend Clinical Manager, Home Health

This is a fully remote position, open to applicants in Kentucky.
• Manage clinical operations at the location, encompassing patient care delivery, staff supervision, documentation quality assurance, and adherence to regulatory standards, while working onsite in-office.
• Assess referrals, evaluate admission suitability, assign clinicians, and verify that Plans of Care align with patient requirements and agency protocols.
• Mentor, assist, and instruct clinicians; aid in goal-setting, care planning, and clinical decision-making; and remain accessible during operating hours for clinical support.
• Ensure that clinical documentation, audits, and billing comply with Medicare, payer, and company regulations; oversee case management quality and outcomes.
• Engage in the recruitment, training, performance assessment, coaching, and corrective actions for clinical personnel.
• Provide ongoing staff training based on documentation evaluations, utilization review outcomes, and performance enhancement data.
• Facilitate communication among physicians, team members, and caregivers to enhance care coordination, discharge planning, and achievement of outcomes.
• Contribute to quality improvement initiatives, data tracking, budgeting efforts, marketing projects, and community relationship building.
• Deliver direct patient care on a limited basis in exceptional or unforeseen circumstances and serve as Branch Director in their absence.
• Execute additional duties to bolster clinical operations and meet organizational objectives.
• Valid state license as a Registered Nurse.
• Current CPR certification.
• Experience in Home Health.
• Proficiency in Homecare Homebase (software).
• Familiarity with OASIS.
• Possession of a valid driver's license, auto insurance, and reliable transportation.
• Minimum of two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or similar setting.
