Virtual Primary Care Physician – Full Time

This is a fully remote position, open to applicants in New Jersey, +1 more state.

📋 Description

• Deliver telehealth services to patients within the primary care network.

• Conduct outreach to patients who may benefit from a visit to primary care.

• Collaborate closely with primary care and population health teams.

• Offer patient-centered care, ensuring a strong commitment to enhancing the patient experience.

• Perform annual screenings and/or wellness visits with assigned patients.

• Fulfill additional required screenings and care tasks.

• Evaluate chronic disease management requirements and support patients in improving their disease management.

• Communicate with continuity primary care providers, playing a vital role in re-engaging and caring for patients.

⛳️ Requirements

• MD or DO degree, or an equivalent qualification.

• Board certified, or eligible for certification, in Family Medicine or Internal Medicine.

• A minimum of 3 years of experience in primary care.

• Licensure in both Pennsylvania and New Jersey is required, or the ability to obtain it by the start date.
