📋 Description

• Evaluate the daily caseload and prepare the necessary technology and tools for patient services.

• Utilize modern technology to manage the daily schedule effectively.

• Collaborate with clinical in-home team members who conduct visits at patients’ residences.

• Establish warm connections with a diverse range of patients.

• Gather pertinent patient information and perform a variety of services.

• Conduct virtual wellness visits for adults and/or children.

• Carry out health risk assessments, medication reviews, and cognitive screenings.

• Educate patients regarding health and well-being topics.

• Assess vital signs and coordinate blood draws with in-home team members.

• Provide care coordination and management, which includes preventive care interventions, medication management, specialist referrals, community resources, and documentation.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner license in one of the following states: KY, MI, OH, IL.

• Previous experience conducting wellness visits.

• High levels of professionalism, integrity, accountability, confidentiality, care, and compassion.

• Willingness to engage in last-minute problem solving and innovative thinking.

• Proficient with technology and comfortable using laptops or mobile devices for charting and HIPAA secure messaging applications for care coordination.

• Strong written and verbal communication abilities.

• Capability to work independently or as part of a team.

• Successfully pass a national background check and valid clinical license verification.

• Experience with Medicare and/or Medicaid Annual Wellness Visits.

• Virtual experience as a Family Nurse Practitioner.

• Current prescriptive authority in the state(s) where licensed to practice as a Nurse Practitioner.
