Virtual Clinician – Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Facilitate approximately 30-minute online Video Visits with members of Medicare Advantage, Medicaid, and certain commercial plans.

• Evaluate medications and medical history during these Video Visits.

• Inform members about their health and address their inquiries.

• Assist in closing gaps in care and connecting individuals to the appropriate services.

• Provide 40 hours of availability each week, achieving over 320 capacity units per month.

• Maintain a high visit completion rate with over 220 completed visits monthly.

• Engage in regular training and adhere to training, affiliation, and compliance standards.

• Obtain training and credentialing for health plan clients and lines of business in the primary coverage state(s).

• Attend regular meetings with management, the team, and clinical staff.

• Meet Key Performance Indicators (KPIs) and metrics related to operational, clinical, and member experience standards set by Signify Health.

• Conduct telehealth services exclusively from a private and adequately connected environment.

• Protect health plan member privacy and operate required technology, including computers, phones, iPads, and other electronic devices.

⛳️ Requirements

• Must be a board-certified nurse practitioner or physician assistant.

• Possess active state licenses in the relevant coverage areas.

• Capability to acquire and maintain multi-state licenses as necessary in the coverage area.

• FNP or AGNP certification is preferred for nurse practitioners.

• Previous experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth delivery, or a similar format is preferred.

• Availability to provide 40 hours of service weekly.

• Capability to consistently deliver over 320 capacity units each month.

• Consistently complete more than 220 visits monthly.

• Commitment to regular training and maintenance of necessary credentialing and compliance.

• Reliable broadband connectivity with enabled video and audio.

• Quiet setting with a neutral background for conducting visits.

• Ability to maintain health plan member privacy in accordance with CVS Health guidelines.

• Strong verbal, written, and electronic communication skills.

• Ability to work independently and practice autonomously.

• Proficient in information management and technology.

• Capable of interacting remotely with members, sitting, writing, operating computers and phones, speaking clearly, and hearing member responses.

• Able to view and read information on computer screens, iPads, and other electronic devices.
