Virtual Clinician, Home Evaluations – Nurse Practitioner or Physician Assistant

This is a fully remote position, open to applicants in Oregon.

📋 Description

• Conduct approximately 30-minute online Video Visits with members enrolled in Medicare Advantage, Medicaid, and select commercial plans.

• Review the medical histories and medications of members.

• Provide education to members regarding their health and address any inquiries.

• Assist in closing care gaps and connect members to the appropriate services.

• Offer 40 hours of availability each week, achieving over 320 capacity units monthly.

• Maintain a high visit completion rate with at least 220 visits completed each month.

• Ensure compliance with Video Visit requirements, including training and affiliation.

• Complete necessary training and credentialing for health plan clients and lines of business in primary coverage states.

• Consistently participate in manager, team, and clinical meetings.

• Meet operational, clinical, and member experience KPIs and metrics set by Signify Health.

• Conduct Video Visits exclusively through remote telehealth services.

• Utilize a computer, phone, iPad, and other electronic devices while engaging with members remotely.

⛳️ Requirements

• Availability for full-time work.

• Must be a board-certified nurse practitioner or physician assistant.

• Hold active state licenses in the coverage areas.

• Capability to obtain and maintain multi-state licenses in the required coverage areas.

• FNP or AGNP certification is preferred for nurse practitioners.

• Complete training and credentialing for all health plan clients and lines of business in the primary coverage state(s).

• Preferred experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth delivery, or similar formats.

• Meet minimum requirements for remote visit delivery, including broadband connectivity with enabled video and audio.

• Have access to a private, quiet setting with a neutral background for conducting visits.

• Ability to uphold member privacy in accordance with CVS Health guidelines.

• Strong verbal, written, and electronic communication skills.

• Capacity to work independently and practice autonomously.

• Proficiency in information management and technology.

• Ability to sit, write, operate a computer and phone, communicate clearly, and hear member responses.

• Capability to view and read screens on computers, iPads, and other electronic devices.
