Virtual Clinician, Home Evaluations, Nurse Practitioner or Physician Assistant

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Conduct virtual 'Video Visits' with members of health plans.

• Operate from a private space equipped with a reliable internet connection.

• Offer 40 hours of availability each week, consistently achieving over 320 capacity units monthly.

• Maintain a robust visit completion rate, consistently completing more than 220 visits each month.

• Uphold training, affiliation, and compliance with Video Visit standards.

• Regularly participate in manager, team, and clinical meetings.

• Be accountable for Signify Health's operational, clinical, and member experience key performance indicators and metrics.

⛳️ Requirements

• Full-time, board-certified nurse practitioners or physician assistants.

• Trained and credentialed for all health plan clients and lines of business in primary coverage states.

• Hold active state licenses in coverage areas (FNP, AGNP certification preferred for NPs).

• Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format is preferred.

• Strong verbal, written, and electronic communication skills.

• Capability to work independently and practice autonomously.

• Proficient in information management and technology.
