Virtual Care Referral Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Provide assistance to members through telephone and messaging as part of a virtual primary care team, ensuring that all external and internal referrals and orders—such as specialist referrals, imaging requests, prior authorizations, and other orders—are completed and routed correctly to the relevant external provider.

• Support patients by providing maps, facility locations, and direct dial numbers.

• Coordinate medical records to guarantee that all necessary documentation, imaging, and procedural results are available to the external provider prior to the patient's appointment and are added to the internal medical record for provider assessment.

• Aid in the management of electronic medical records and handle records requests.

• Assist providers in completing patient forms (e.g., Medical Opinion forms, SSI Disability forms) when suitable.

• Collaborate effectively with staff, including providers, the care team, and patient support, compliance, and others.

• Demonstrate the ability to thrive in a fast-paced, dynamic environment with competing priorities.

• Relevant work experience in a physician's office or medical institution, with knowledge of managed care and primary care regulations and practices.

• Ability to operate independently and utilize resources efficiently to address inquiries.

• Competence in multitasking and managing several priorities simultaneously.
