Triage RN

This is a fully remote position, open to applicants in Colorado, +2 more states.

📋 Description

• Respond to triage calls from various Assisted Living (AL) and Memory Care (MC) locations during non-business hours and on weekends.

• Accurately document any guidance and essential information in the Electronic Health Record (EHR) for the site clinical team to follow up and implement.

• Compile activity and incident reports for the site Clinical Team(s) to analyze call volume and identify potential training needs for staff to ensure safe and efficient operations.

• Must be willing to engage in any investigations or surveys related to after-hours calls.

• Foster a positive, team-oriented atmosphere and promote a healthy work environment for those who depend on their expertise.

⛳️ Requirements

• Must maintain a current state license as a Registered Nurse in accordance with state regulations.

• A minimum of 5 years’ experience in home health, assisted living, or long-term care is preferred.

• Strong organizational skills with the ability to initiate and follow through on tasks with minimal supervision.

• Demonstrates a caring attitude and a genuine passion for elderly care.

• Exhibits sound judgment, problem-solving abilities, and decision-making skills.

• Outstanding verbal and written communication skills.

• Familiarity with Electronic Health Records (EHR), specifically Eldermark and Point Click Care.

• Excellent communication skills, coupled with patience and empathy for on-site staff and residents.

• Must possess confidence in clinical knowledge and decision-making.

• A solid understanding of pharmacology, basic first aid, and blood glucose monitoring is essential.

• Prior supervisory experience is a plus.
