Transition Planner

This is a fully remote position, open to applicants in United States.

📋 Description

• Develop and revise an overarching transition plan outline in accordance with the statement of work as necessary.

• Collaborate with the Project Manager to establish inventory criteria and recording methodologies for reuse planning purposes.

• Create conceptual advance move planning activities that the facility needs to address.

• Assess both positive and negative risks associated with specific transition issues for the local MTF in conjunction with the Project Team.

• Identify all occupancy milestones, including equipment procurement and installation timelines.

• Elaborate on implementation plans, move sequencing, preparation, and setup.

• Compile a comprehensive manual of detailed move plans for each location.

• Lead the development of an aggressive pre-move purge plan process with the MTF.

• Issue a 90-day countdown plan for the MTF to keep the project team aligned.

• Design an orientation and training program to ensure staff are fully acquainted with the new site, including fire, safety, security, low voltage, communications, and other building systems, services, and patient care systems.

• Coordinate the training schedule for new equipment with the MTF.

• Formulate a relocation execution plan: Define staff access control for the new area and key turnover for the old area, including scheduling the programming of electronic keys as directed by the Facility Manager.

• Identify, plan for, and coordinate the relocation or replacement of all equipment that is either leased or must be managed by the vendor (such as copy machines, lab analyzers, Q-flow systems, or pharmacy automated pill dispensers).

• Collaborate with local Communications operations to assign new phone numbers and transfer existing ones.

• Work with the Facility Manager to identify and implement any required Interim Life Safety Measures (ILSMs) during the transition.

• Develop a post-move aseptic cleanup plan.

• Create plans for the excessing and turnover of old furniture and equipment not slated for reuse.

• Manage any transition-related services, such as temporarily adjusting traffic signage during the move and informing them of changes in operating hours.

• Coordinate the confirmation of all commodity delivery dates with the PDT.

• Arrange for the delivery and emptying of recycling dumpsters to manage increased trash during the installation of equipment and furniture.

• Develop a lessons-learned and Post Occupancy Evaluation (POE) plan to be immediately implemented in the transition planning for the next construction phase and, if applicable, the option-year contract.

• A lessons-learned template and examples of POE will be provided.

• Ensure that room signage is complete and accurate, including the names of staff members.

• Develop the Integrated Master Schedule for IO: For each construction phase of the project, the Transition Planner will create an IO Master Schedule that includes, at a minimum, the Project Milestones.

• Support the IO-PM to ensure that activities related to the Master Construction Schedule are integrated into the project's Integrated Master Schedule and are regularly updated and distributed to the PDT monthly or as required for interim updates.


⛳️ Requirements

• 6 years of experience in Healthcare Move and Transition planning.

• 6 years of experience in Federal, DOD, or Military projects.

• 3-5 years of experience in project management.

• Proficiency in reading AutoCAD drawings is required.

• Must reside in the lower 48 states and be available for travel as needed.

• Experience in OCONUS transition planning is required.

• BA/BS in a related field.

• Knowledge of MS Project Scheduling.

• Must be able to pass military background checks to access bases.
