📋 Description

• Start your day by assessing your case load and preparing your technology.

• Review your daily schedule and collaborate with our clinical in-home team members.

• Establish a warm and welcoming rapport with a diverse array of patients.

• Perform comprehensive health risk assessments, medication evaluations, and cognitive screenings.

• Deliver extensive care coordination and management, which includes preventive care interventions and referrals to specialists.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner License in California.

• Credentialed Medicaid provider for the state of California (Medi-Cal).

• Consistently demonstrates the highest levels of professionalism, integrity, accountability, confidentiality, care, and compassion to deliver high-quality health services.

• Willingness to operate in a revolutionary environment that may require last-minute problem solving and innovative thinking.

• Technologically proficient and comfortable utilizing tools such as laptops or mobile devices for charting and HIPAA-secured messaging apps for care coordination.

• Strong written and verbal communication skills.

• Ability to work independently or collaboratively within a team environment.

• Successfully pass a national background check and possess a valid clinical license.
