This is a fully remote position, open to applicants in Massachusetts.
• Perform thorough telephonic and virtual clinical evaluations, which include the analysis and interpretation of biometric, RPM, and IVR data with trend assessments.
• Create, implement, and modify personalized nursing care plans that align with patient objectives, evidence-based practices, and organizational protocols.
• Utilize Motivational Interviewing techniques to engage patients and promote behavior modification, adherence, and self-management.
• Recognize clinical risks, changes in patient conditions, and urgent scenarios; escalate these issues as necessary and in accordance with clinical guidelines.
• Collaborate with interdisciplinary care teams (both internal and external) to ensure seamless care coordination, referrals, and transitions throughout the healthcare continuum.
• Implement proactive measures aimed at minimizing hospital admissions, emergency room visits, and overall healthcare costs.
• Accurately, promptly, and comprehensively document all patient interactions in the electronic health record (EHR).
• Engage in quality improvement efforts, chart audits, and performance evaluations to improve clinical outcomes and care delivery.
• Conduct care management tasks that align with client-specific workflows, performance standards, and service-level expectations.
• Adhere to all clinical, regulatory, HIPAA, and organizational standards for compliance.
• A nursing degree from an accredited college or university (ADN or BSN required; BSN preferred).
• At least 5 years of extensive clinical nursing experience.
• A minimum of 3 years of experience in one or more areas: Care management or care coordination, Home health or transitions of care, Telehealth or virtual care, Chronic disease management (e.g., cardiac, pulmonary, endocrine).
• Proficiency in computer skills, including EHR documentation, navigating various systems, and resolving basic technical issues.
• Willingness to work designated time zones and schedules based on client requirements.
• An active, unrestricted RN license, Compact (NLC) license, and/or California RN license is required, depending on the assignment.
