Telephonic Nurse Care Associate

This is a fully remote position, open to applicants in Wisconsin.

📋 Description

• Perform telephonic outreach to patients participating in Remote Patient Monitoring (RPM) and chronic care programs.

• Assess biometric alerts and recognize out-of-range readings; escalate clinical issues as per established protocols.

• Deliver patient education and coaching to enhance adherence to care plans and lifestyle modifications.

• Precisely record patient interactions in EMR/RPM systems in accordance with clinical and regulatory standards.

• Collaborate with clinical management, RN Care Managers, healthcare providers, and interdisciplinary care teams to ensure coordinated patient care.

• Assist in patient engagement initiatives aimed at enhancing monitoring compliance and improving clinical outcomes.

⛳️ Requirements

• Current LPN/LVN license in good standing.

• Over 5 years of clinical experience, including at least 3 years in care coordination, telehealth, home care, or chronic disease management.

• Preferred experience in supporting patients with conditions like CHF, COPD, diabetes, or hypertension.

• Excellent telephonic communication, patient education, and motivational interviewing abilities.

• Familiarity with EMR/EHR systems and comfort operating in a technology-enabled care environment.

• Capability to work autonomously within a structured remote clinical workflow.
