Telephonic Nurse Care Associate

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Perform telephonic outreach to patients participating in Remote Patient Monitoring (RPM) and chronic care initiatives.

• Analyze biometric alerts to pinpoint out-of-range readings and escalate clinical issues in line with established protocols.

• Deliver patient education and coaching to enhance adherence to care plans and lifestyle suggestions.

• Ensure accurate documentation of patient interactions within EMR/RPM systems, adhering to clinical and regulatory guidelines.

• Work collaboratively with clinical management, RN Care Managers, healthcare providers, and interdisciplinary care teams to facilitate coordinated patient care.

• Contribute to patient engagement initiatives aimed at boosting monitoring compliance and enhancing clinical outcomes.

⛳️ Requirements

• Current LPN/LVN license in good standing.

• Over 5 years of clinical experience, with at least 3 years in care coordination, telehealth, home care, or chronic disease management.

• Preferred experience in supporting patients with conditions such as CHF, COPD, diabetes, or hypertension.

• Exceptional telephonic communication, patient education, and motivational interviewing abilities.

• Familiarity with EMR/EHR systems and proficiency in a technology-driven care environment.

• Capability to operate independently within a structured remote clinical workflow.
