Telephonic Nurse Care Associate

This is a fully remote position, open to applicants in Maryland.

📋 Description

• Perform telephonic clinical outreach to individuals participating in Remote Patient Monitoring (RPM) and chronic care programs.

• Assess biometric alerts and pinpoint out-of-range readings; escalate clinical issues following established protocols.

• Deliver patient education and coaching to enhance adherence to care plans and lifestyle recommendations.

• Thoroughly document patient interactions in EMR/RPM systems while adhering to clinical and regulatory standards.

• Collaborate with clinical management, RN Care Managers, healthcare providers, and interdisciplinary care teams to ensure comprehensive patient care.

• Assist in patient engagement initiatives aimed at boosting monitoring compliance and improving clinical outcomes.

⛳️ Requirements

• Must possess an active LPN/LVN license in good standing.

• A minimum of 5 years of clinical experience, with at least 3 years in care coordination, telehealth, home care, or chronic disease management.

• Preferred experience in supporting patients with conditions such as CHF, COPD, diabetes, or hypertension.

• Excellent telephonic communication skills, alongside patient education and motivational interviewing abilities.

• Familiarity with EMR/EHR systems and comfort in a technology-enhanced care environment.

• Capability to operate independently within a structured remote clinical workflow.
