This is a fully remote position, open to applicants in Maryland.
β’ Perform telephonic clinical outreach to individuals participating in Remote Patient Monitoring (RPM) and chronic care programs.
β’ Assess biometric alerts and pinpoint out-of-range readings; escalate clinical issues following established protocols.
β’ Deliver patient education and coaching to enhance adherence to care plans and lifestyle recommendations.
β’ Thoroughly document patient interactions in EMR/RPM systems while adhering to clinical and regulatory standards.
β’ Collaborate with clinical management, RN Care Managers, healthcare providers, and interdisciplinary care teams to ensure comprehensive patient care.
β’ Assist in patient engagement initiatives aimed at boosting monitoring compliance and improving clinical outcomes.
β’ Must possess an active LPN/LVN license in good standing.
β’ A minimum of 5 years of clinical experience, with at least 3 years in care coordination, telehealth, home care, or chronic disease management.
β’ Preferred experience in supporting patients with conditions such as CHF, COPD, diabetes, or hypertension.
β’ Excellent telephonic communication skills, alongside patient education and motivational interviewing abilities.
β’ Familiarity with EMR/EHR systems and comfort in a technology-enhanced care environment.
β’ Capability to operate independently within a structured remote clinical workflow.
