This is a fully remote position, open to applicants in Connecticut.
β’ Perform telephonic clinical outreach to patients participating in Remote Patient Monitoring (RPM) and chronic care programs.
β’ Assess biometric alerts and recognize out-of-range readings; escalate clinical issues following established protocols.
β’ Deliver patient education and coaching to enhance adherence to care plans and lifestyle recommendations.
β’ Precisely document patient interactions in EMR/RPM systems while adhering to clinical and regulatory standards.
β’ Collaborate with clinical management, RN Care Managers, providers, and interdisciplinary care teams to ensure coordinated patient care.
β’ Assist in patient engagement initiatives aimed at enhancing monitoring compliance and improving clinical outcomes.
β’ Valid LPN/LVN license in good standing.
β’ Over 5 years of clinical experience, including at least 3 years in care coordination, telehealth, home care, or chronic disease management.
β’ Preferably experienced in supporting patients with conditions such as CHF, COPD, diabetes, or hypertension.
β’ Excellent telephonic communication, patient education, and motivational interviewing abilities.
β’ Familiarity with EMR/EHR systems and comfort in a technology-driven care environment.
β’ Capability to work independently within a structured remote clinical workflow.
