This is a fully remote position, open to applicants in New Hampshire.
• Act as the main point of contact for patients who have been newly referred.
• Perform outbound calls to newly enrolled prospective patients to describe AMC Health's Remote Patient Monitoring programs, services, and advantages.
• Interact with patients via telephone to build rapport, answer inquiries, and promote participation and adherence to monitoring programs.
• Accurately and thoroughly document patient information in the electronic health record (EHR).
• Work collaboratively with nurse care managers and monitor hospitalizations as necessary.
• Address inbound inquiries from patients, clients, and providers, and effectively assess service needs.
• Current CNA certification or an equivalent credential (e.g., Pharmacy Technician), in good standing.
• Preferred: Completion of a Medical Assisting program accredited by CAAHEP or ABHES.
• Minimum experience qualifications: At least six (6) months of experience in general duties as a Medical Assistant OR three (3) years of experience in a clinic or similar healthcare environment with direct patient interaction.
• Familiarity with medical terminology and prior experience utilizing electronic health record (EHR) systems.
• Strong technical skills, including the ability to assist patients remotely with health devices and resolve basic issues.
• Capacity to type a minimum of 50-60 words per minute while conversing with patients by phone.
• Must be able to work schedules aligned with client-specific time zone requirements, as dictated by business needs.
