This is a fully remote position, open to applicants in Kentucky.
• Act as the main contact for patients who have been newly referred.
• Engage in patient outreach, registration, enrollment, and telephonic communication.
• Assist patients dealing with chronic illnesses such as CHF, COPD, diabetes, and hypertension.
• Establish trust and show empathy towards patients.
• Clearly communicate the services provided by Remote Patient Monitoring (RPM).
• Guarantee efficient and compliant enrollment procedures.
• Work in conjunction with nurse care managers and other members of the clinical team.
• Accurately record patient information in the Electronic Health Record (EHR) system.
• Exceptional organizational, time management, and multitasking abilities.
• Outstanding verbal and written communication skills with a keen eye for detail.
• Proven capability to actively listen and engage with patients compassionately and professionally.
• Ability to work autonomously in a remote setting while remaining accountable for assigned responsibilities.
• Strong problem-solving and critical-thinking abilities, with a commitment to following issues to resolution.
• Demonstrated ability to build rapport and sustain productive relationships in a telephonic and remote environment.
• Proficient in navigating multiple systems and technologies concurrently.
• Bilingual (Spanish) is preferred.
• Completion of a Medical Assisting program accredited by CAAHEP or ABHES is preferred.
• A minimum of six (6) months of experience performing general tasks as a Medical Assistant OR three (3) years of experience in a clinic or similar healthcare environment with direct patient interaction.
• Familiarity with medical terminology and prior experience using electronic health record (EHR) systems.
• Strong technical skills, including the ability to assist patients remotely with health devices and troubleshoot basic issues.
• Capability to type at least 50-60 words per minute while conversing with patients on the phone.
• Must be able to work schedules that align with client-specific time zone requirements, as needed by business operations.
• Active CNA certification or an equivalent certification (e.g., Pharmacy Technician), in good standing.
