Telehealth Preventative Primary Care Advanced Practice Provider – NP/PA

This is a fully remote position, open to applicants in Oregon.

📋 Description

• Primarily conduct telehealth video consultations, with occasional in-home visits to local members.

• Carry out thorough assessments and deliver evidence-based care emphasizing prevention, chronic condition management, medication optimization, and health maintenance.

• Recognize and address any gaps in care.

• Collaborate with both external and internal care team members.

• Play a role in creating personalized, holistic care plans.

• Leverage integrated technology and AI-driven workflows to alleviate administrative tasks and enhance meaningful interactions with members.

• Communicate with compassion and clarity, building trust and reassurance with members and their caregivers.

• Work in partnership with primary care providers, specialists, pharmacists, nurses, care coordinators, caregivers, and families.

⛳️ Requirements

• Current Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.

• Over 3 years of outpatient clinical experience, preferably in primary care, family medicine, internal medicine, or geriatrics.

• Maintain an active and clear NP/PA license.

• Willingness to obtain additional licenses as required within the first 90 days.

• Hold an active BLS certification at the time of hire.

• Comfortable providing care through video telehealth and utilizing electronic documentation systems.

• Understanding of managed care models, including STARS/HEDIS, and the ability to identify care gaps.

• Experience in conducting comprehensive or preventive care visits with Medicare populations.

• Background in supporting older adults and/or individuals with complex medical or social needs.
