This is a fully remote position, open to applicants in United States.
• Perform telehealth assessments for members with intricate medical, medication, or care coordination requirements.
• Assess and prioritize clinical needs, translating them into actionable next steps for the care team.
• Collaborate with members' primary care physicians and specialists to ensure continuity of care.
• Review and approve care plans created in conjunction with RNs, LCSWs, and care navigators.
• Act as the escalation point for clinical issues and respond on the same day they arise.
• Direct care plans that address medical necessities and social determinants of health.
• Create and uphold clinical protocols, escalation criteria, and scope-of-practice guidelines.
• Facilitate training for nurses and care navigators.
• Collaborate with Compliance and program leadership regarding clinical quality, documentation standards, and new program initiatives.
• Ensure accurate and timely documentation for every evaluation and care plan review.
• Maintain licensure and credentialing while adhering to telehealth, HIPAA, and consent regulations.
• Propel clinical quality and program KPIs, such as care plan reviews, escalation responses, outcomes, protocol implementation, and documentation precision.
• Availability to work Monday–Friday, from 9am to 6pm ET.
• Residing in the United States and authorized to work in the U.S. without employer sponsorship.
• MD or DO with board certification in Internal Medicine, Family Medicine, or Geriatrics.
• Active, unrestricted medical license in at least one U.S. state.
• Willingness to obtain additional state licenses as the program grows.
• A minimum of 3 years of post-residency clinical experience.
• Experience in a collaborative care model involving nurses, social workers, or care coordinators.
• Strong comprehension of social determinants of health and their impact on outcomes.
• Proficient with telehealth platforms, EHRs, and care management tools.
• Comfortable navigating ambiguity and rapid iteration.
• Eligibility for the Interstate Medical Licensure Compact (IMLC) or possession of multiple active state licenses.
• Active DEA registration.
• Experience with Medicare, dual-eligible, or older adult demographics.
• Background in transitional care, value-based care, or population health.
• Experience in a startup environment or new program launch.
• Bilingual skills, preferably in Spanish.
