Telehealth Nurse Practitioner

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Perform thorough health evaluations through telehealth services

• Complete documentation for HCC risk adjustment

• Address HEDIS care gaps during patient appointments

• Record visits utilizing ICD-10 and CPT II coding

• Assess medications, medical history, and preventive care necessities

• Provide clear care plans along with follow-up instructions

⛳️ Requirements

• Valid Nurse Practitioner license in Louisiana

• Minimum of 1 year experience in Family or Internal Medicine

• Experience with Medicare and Medicaid patients

• Knowledge of HEDIS and risk adjustment processes

• Required enrollment as a provider for Medicare and Medicaid

• Ability to use telehealth platforms independently
