Telehealth Nurse Practitioner

This is a fully remote position, open to applicants in Iowa.

📋 Description

• Perform thorough health evaluations via telehealth.

• Record HCC risk adjustments during patient consultations.

• Address HEDIS (quality measures) care gaps.

• Assess medical history, current medications, and preventive requirements.

• Utilize ICD-10 and CPT II codes for visit documentation.

• Provide concise care plans and follow-up instructions.

• Ensure precise documentation for quality assurance programs.

⛳️ Requirements

• Valid Nurse Practitioner license in Iowa.

• Minimum of 1 year experience in Family Medicine or Internal Medicine.

• Familiarity with Medicare and Medicaid patient populations.

• Knowledge of HEDIS and risk adjustment processes.

• Required enrollment as a provider for Medicare and Medicaid.

• Proficient in delivering care using telehealth platforms.
