This is a fully remote position, open to applicants in United States.
• Conduct virtual consultations for chronic condition monitoring, medication reconciliation, and follow-up care.
• Evaluate members' health conditions, recognize clinical and social needs, and document clear plans with actionable next steps.
• Share findings and recommendations with members' primary care providers.
• Carry out follow-up visits after discharges from hospitals or emergency departments.
• Deliver medication reviews and educate on warning signs.
• Identify members at risk for readmission and coordinate follow-up with the care team.
• Act as a clinical resource for RNs, LCSWs, and care navigators.
• Address clinical escalations on the same day they are identified.
• Assist in training on clinical red flags, medication fundamentals, and escalation criteria.
• Provide input on workflows, protocols, and tools as the program develops.
• Complete precise, same-day documentation for each visit.
• Maintain licensure, board certification, and necessary collaborative practice agreements.
• Adhere to telehealth, HIPAA privacy, and consent regulations.
• Drive clinical quality, productivity, and compliance KPIs.
• Availability during core business hours, Monday–Friday, Eastern Time.
• Residing in the United States and authorized to work in the U.S. without employer sponsorship.
• Board certification as a Family (FNP) or Adult-Gerontology (AGNP) Nurse Practitioner.
• Active, unrestricted APRN license in at least one U.S. state.
• Willingness to secure additional state licenses as the program grows.
• Over 3 years of NP experience in primary care, internal medicine, geriatrics, or transitional care.
• Comfortable discussing chronic conditions, medications, and treatment plans in straightforward terms with older adults.
• Proficient with telehealth platforms, EHRs, and care management tools.
• Able to navigate ambiguity and rapid changes effectively.
• Additional licensure in multiple states, active DEA registration, experience with Medicare/dual-eligible/older adult populations, telehealth/home-based/transitional care experience, or bilingual in Spanish are advantageous.
