This is a fully remote position, open to applicants in Wisconsin.
• Deliver high-quality and equitable value-based primary care to Medicare adults through telemedicine.
• Conduct same-day sick consultations, manage acute escalations, perform post-discharge visits, oversee chronic disease management, and execute annual wellness evaluations.
• Independently assess patients and collaborate with physicians based on individual patient requirements.
• Apply evidence-based practices to deliver virtual healthcare.
• Carry out annual wellness visits and health risk assessments with precise and thorough documentation.
• Perform consultations via video and phone.
• Manage acute escalations received from the call center and facilitate subsequent management steps.
• Coordinate care with primary care providers, licensed medical social workers, behavioral health experts, specialty care, and testing facilities.
• Monitor patient outcomes with a focus on population health and engage in regular outcome reviews.
• Provide education to patients and caregivers on preventative care, lifestyle changes, and self-management techniques.
• Adapt to the changing telehealth landscape and support Oak Street’s care model.
• Acquire and maintain multiple state licenses as per business requirements.
• Carry out additional responsibilities as assigned.
• Report to the Central Telehealth Medical Director and collaborate closely with operational leadership.
• Master of Science in Nursing, Doctor of Nursing Practice, or graduate of an NP Certificate Program.
• National certification in at least one of the following: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
• Active, non-probationary state Nurse Practitioner license.
• Capacity to obtain new licenses as necessary.
• Authorization to work in the US.
• Willingness to acquire and maintain multiple state licenses, including RN, NP, CSR, and DEA, as per business needs.
• 3–5 years of experience in primary care, encompassing internal medicine, geriatrics, or family medicine.
• Preferred 1–2 years of experience in virtual care.
• Strong preference for licensure in AL, AR, AZ, CO, IA, IL, IN, LA, NM, OK, TN, or TX.
• Openness to working across MT, ET, and CT time zones.
• Strongly preferred Spanish language proficiency.
• Competency in using telehealth platforms and technology.
• Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments.
• A passion for teamwork and interdepartmental collaboration.
