Supportive Care Case Manager, Palliative Care RN

📋 Description

• Required to have experience in Palliative Care.

• This is a full-time remote position, allowing you to work from your designated home office with a Monday to Friday schedule, adaptable to accommodate patient availability.

• Work collaboratively with the Operations Manager, Clinical Services Manager, Nurse Practitioners, Social Workers, and Case Managers to review patient cases, identify barriers, and coordinate advanced patient care plans.

• Facilitate complex discussions and conduct thorough, empathetic telephonic conversations to assist patients with potentially deteriorating conditions by educating them on treatment options, ensuring their care aligns with their preferences, and guiding them to suitable resources.

• Lead Goals of Care (GOC) discussions, translating patient wishes into actionable care plans; document and update GOC to ensure consistency across the entire healthcare team, including Nephrologists, Dialysis Centers, and IKC RNs.

• Provide education to patients on Advanced Directives (MOLST/POLST) and promote their completion to avert unwanted aggressive interventions.

• Evaluate symptom burden (pain, anxiety, dyspnea) and collaborate with providers to implement nursing interventions aimed at enhancing Quality of Life (QOL).

• Demonstrate strong clinical judgment with the ability to recognize the progression of chronic illnesses, identify critical changes to proactively address those at risk of rapid decline or readmission, and intervene immediately for transitions to supportive care.

• Assess patient readiness and appropriateness for palliative care or hospice services, facilitating smooth transitions to these external services.

• Educate patients on curative treatments, palliative support, and hospice care to mitigate access barriers.

• Exhibit exceptional active listening and motivational interviewing skills; capable of discussing sensitive topics (death, dying, prognosis) with empathy and clarity.

• Ability to operate independently in a remote setting, managing a caseload of high-complexity patients with minimal oversight.

• Facilitate communication among patients and their families, and develop and/or update advance care plans.

• Provide comfort case management for patients when palliative care is unavailable or when they are not ready to cease dialysis, yet wish to pursue comfort care.

• Coordinate care for patients, including care transitions, management of complex or at-risk patients, addressing ongoing needs, and establishing treatment plans in collaboration with the care team.


⛳️ Requirements

• A minimum of 2+ years of experience as an RN in Palliative medicine is required.

• At least five (5) years of experience as an RN is required.

• 2+ years of experience in care plan creation, proactive discharge planning, and utilization.

• A current RN License is mandatory; a BSN is preferred.

• Current certification in Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) is required.

• Proficiency in EMR systems (Cerner), Microsoft Office (Teams, Excel), and virtual communication platforms is essential.

• A home office with internet connectivity of at least 1MB upload and 1MB download speed is required.
